LEAGUE CERTIFICATION

MEMBERSHIP APPLICATION CARDS: EVERY bowler MUST complete their membership application and pay their dues BEFORE they complete their first series in league competition. This includes substitute bowlers regardless of how many games they may bowl. Games bowled by unsanctioned bowlers are subject to forfeiture according to USBC regulations. Please ask every person to fill out their application card completely and correctly. Remember, their sanction cards will be mailed directly to their home addresses, so check for any errors. Please be sure each person uses the FULL NAME of the league through which the application is being submitted and the league name through which their fees are being paid. Every bowler in your league must fill out a certification application, whether they are paying dues through your league or not. Please note the amount of dues paid on the card – either $0, $6, $7 or $20.

If the League Roster that is returned to you does NOT have all of the bowlers' names on it, please contact the Association Manager at once. At the end of the season, you will be sent an average verification sheet to submit the final averages to the Association for publication in next year's average book.

League Applications with the completed membership application cards must be sent to the Association Manager of the Portage County USBC Bowling Association within 3 weeks of your league's starting date. Be sure to furnish ALL of the information on the form. Checks should be made payable to Portage County USBC or PC USBC BA. Please DO NOT send cash through the mail.
